| Abstract The perspective for a cost-effectiveness analysis should be that of the decision maker. In the four decades since these analyses have been used in the health sector the perspective almost invariably has been government or society. This invited editorial introduces the paper by Guico-Pabia and colleagues in the same journal issue. The editorial highlights the paper's a fresh, new perspective: the private employer, based on the employer's pivotal role in structuring most group health insurance. Typically, the employer directly decides what services to cover and the degree and types of controls on their use, and sharing of insurance premiums between the employee and the worker. Indirectly, the employer determines the types of managed care arrangements through the choice of firm and level of control that it permits, e.g., choice of provider, utilization review, and incentives. | |
