| Description This project investigated the effect of the methods and levels of payment for Medicaid nursing facility care on access and quality of care for Medicaid beneficiaries. Economic theory suggests that both access to care and staffing per case-mix-adjusted resident day available to Medicaid residents will be greater where Medicaid payment rates are more closely tied to actual resource use, and where Medicaid rates are greater, other things constant. 1996 data on staffing and patient mix for 14,000 freestanding nursing homes are combined with market area and state variables to estimate the effects of rate methods and average rate on access and quality at the facility, county, and state levels. A second phase of the project will examine changes between 1996 and 1999. The study is especially timely as states consider their response to repeal by the Balanced Budget Act of 1997 of the Boren Amendment, which required that Medicaid rates cover the cost of efficient, economical nursing facility care. | |
