Description
This project examined the application of the Chronic Care Illness Treatment model to the treatment of tobacco dependence. The project produced "Treating Tobacco Use and Dependence as a Chronic Disease: A Planning Guide for Practice Sites in Developing an Office-Based System of Care" as a quality improvement tool for primary care practice sites in assessing current tobacco treatment practices and implementing any Public Health Service Guidelines for treating tobacco use and dependence which may be absent.
